Thank you to everyone who has participated in the Make Your Mark Employee Tile Campaign.
The program has been a huge success with 1400 tiles ordered and more than 1100 members of the Rady Children's family participating. Together we have raised more than $200,000 for the new Acute Care Pavilion.

The program is now closed and we will be working on finalizing the orders and working to create and install them into the new building in time for the September 24th Celebration.
We will be communicating with everyone in the coming months about the grand unveiling ceremonies for each floor taking place during the day long celebration. We hope everyone will be able to join us to view their tiles.

Frequently Asked Questions
1. Where will my donation go?
Your donation will go towards the construction and technology of the new Acute Care Pavilion.
2. Is my donation tax deductible?
Yes, 100% of the cost of the tile is tax deductible.
3. How can I get a receipt for my donation?
If you contribute online, you will be emailed a reciept at the end of the process. If you pay by cash or check, you will recived a thank you and receipt in the mail. If you pay through payroll deduction,you will receive a receipt for all payments that have been made towards your pledge at the end of the year.
4. Where will my tile be displayed?
Your tile will be displayed on floors one through three of the Acute Care Pavilion, depending on the shape of tile you choose, on a specified wall in the Northwest part of the building (near the bridges on floors two and three).
5. I want my tile by a certain department, what floors are the departments on?
Floor 1: Warren Family Surgical Center Floor 2: Peckham Center for Cancer and Blood Disorders Floor 3: Neonatology, NICU, Med/Surgical
6. When will my tile be installed on the wall?
The tiles will be installed right before we open the Acute Care Pavilion, in October 2010
7. What tile shapes will be available for purchase?
Floor 1 - Sea Level: Fish, Starfish or Octopus, Floor 2 - Beach Level: Umbrella, Beach Ball or Surfboard, Floor 3 - Sky Level: Kite, Butterfly or Bird
8. Can I purchase tiles for my family members?
Yes, employees can purchase tiles for their immediate family members (children, grandchildren & parents).
9. Is there a discount if I purchase more than one?
No, all tiles cost $150 each.
10. If I purchase multiple tiles for myself and my family members, can our tiles be placed together, if purchased on the same floor?
Yes, check the box on your order form, indicating you would like that option.
11. What will the tiles say?
a. Your name
b. Year you began work at Rady Children's (optional)
Example: John Smith

12. Is there a limit to what my tiles can say?
Yes, we ask that you limit your tile to names and family names only of 40 characters (including spaces) or less.
You may include the phrases “In Celebration of”, “Celebrating”, “In my/our heart/s”, “With Love”, "Love", “We/I love you”, or "From" as long as it does not go over the 40 characters.
If you are purchasing a tile in recognition of a particular department or team you may list the name of the Department or Team. (Example: RCHSD Foundation).
If you would like to list a year, you may choose to list a single year in a 4 number format (Example: 2010). We are not able to list specific dates such as birthdays.
If you enter text other then the above criteria you may be asked to resubmit your order.

13. Can I buy a tile for my entire family?
Yes

14. How do you confirm that I am a Rady Children's Employee?
The form requires that you list your employee number, which can be found on your paycheck stub.
15. I work at Rady Children's but am employed by a different company, can I purchase tiles?
Yes, we are offering the opportunity to everyone in the Rady Children's family. However, since you are not paid through hospital payroll you must select cash, check or credit card for your payment option.
16. But I don't have an employee number, what should I use?
Please enter the following depending on your status:
If you are a Rady Children's Auxiliary Member please enter Auxiliary in the employee number field and your unit name in the department field.
If you are a Rady Children's affiliated physician please enter Physician in the employee number field.
If you are a contracted employee please enter Contract -"Your employer name" in place of an employee number.
If you are a Rady Children's Volunteer please enter Volunteer in the employee number field.
17. Are the tiles different sizes?
There will be nine different shapes, approximately all the same size.
18. I am not able to order the tile that I want, why?
There are a limited number of tiles for each shape, once a tile has reached its limit, that tile will no longer be available.
19. If I elect to have my pledge taken out of my paycheck, when will the first deduction take place?
Your first deduction will be taken out of your paycheck within 2-4 weeks of placing your order.
20. How do I check my balance paid on my pledge?
You can check your balance two different ways: Look at your paycheck stub, which will have a year-to-date balance paid OR call the Payroll Department at 858.966.4988
21. What if my employment ends with Rady Children's before my pledge is paid?
The remaining balance of your pledge will be deducted from your final paycheck.
